Pre-test forms help teachers collect participant information before they start the test. This feature is particularly useful for public tests or participants who are not yet in a class.
Enable Information Collection Form
- Open the test you are editing.
- Open the Advanced Settings section.
- Find the Pre-test form section.
- Enable Information collection form.
- Enter the field name in the Display label box, such as:
- Full name.
- Student ID.
- Class.
- Email.
- Phone number.
- Work unit.
- Select the appropriate data type.
- Turn on Required if participants must provide the information.
- Select Add field to include more content.
- Review the Form before publishing.
Field types
Single line (text)
Used to collect short content on a single line.
Example: Full name, student ID, class name, or unit.
Multi-line
Used when participants need to enter long content.
Example: Address, notes, reason for participation, or additional information.
Used to collect email addresses. The system can check the format before allowing the user to continue.
Example: name@example.com.
Number
Used only for numeric data.
Example: Exam number, years of experience, or quantity.
Note: Do not use this type for data that may start with a zero, such as student IDs or phone numbers.
Phone
Used to collect the participant's phone number.
Date
Allows participants to select a specific date.
Example: Date of birth, registration date, or course participation date.
Dropdown list
Allows participants to select a value from a pre-set list.
Example:
- Class 10A, 10B, or 10C.
- Hanoi, Da Nang, or Ho Chi Minh City campus.
- Basic, Intermediate, or Advanced level.
This field type helps keep data consistent and limits input errors.
Set required fields
Enable Required for information needed before participants start the test. If a required field is not filled, the participant will not be able to continue.
It is recommended to set identification information such as name, student ID, or email as required. Non-essential information should be left optional.
Add or delete fields
- Select Add field to create an additional information field.
- Click the trash can icon next to a field to delete it.
- Double-check before deleting as the data set for that field may not be recoverable.
Example Form for a public test
Teachers can set up:
- Full name – Single line – Required.
- Email – Email – Required.
- Phone number – Phone – Optional.
- Class/Unit – Dropdown list – Required.
- Notes – Multi-line – Optional.
Notes
- Only collect information that is truly necessary for the purpose of organizing the test.
- Do not request passwords, financial information, or sensitive data.
- Participants should be informed about the purpose of data collection and usage.
- If the test is deployed by class, the system already has account information, so there is no need to ask students to re-enter too much data.
- Set clear field labels and avoid using generic names like “Information 1”.
- You should preview the Form on both computers and phones before publishing.
